When treatment crosses county lines

The investigation should follow that sequence. What did each provider know? Which results were available? Who was responsible for acting on them?

What to tell us during the first conversation

Questions an investigation may address
A review of hospital, surgical, diagnostic, or medication care may examine questions such as:

- Did an abnormal test result reach the person responsible for follow-up?
- What information accompanied a transfer between providers?
- How did the team respond when a complication developed?
- What happened between discharge and an unexpected return to the hospital?
These questions identify possible areas of inquiry. They do not establish malpractice by themselves. A legal review considers the applicable professional standard, whether the care departed from it, and whether that departure caused harm.
